Discription
A slice of liver is mounted. The external surface is coarsely nodular, some of the nodules being 1 cm or larger. The cut surface also reveals a macronodular pattern, the nodules being of varying size, but generally larger than that seen in the micronodular type. The pallor is due to fatty change.
Clinical History
This elderly male had a hernia repaired and an appendectomy, at which time the liver and spleen were found to be enlarged. He denied using alcohol, or ever having been jaundiced. Investigation revealed an elevated bilirubin and liver enzymes, but no hepatitis antigen. He was treated symptomatically for peripheral edema and ascites, but no definite diagnosis was made. Death was attributed to liver failure.
Autopsy Findings
These included gross peripheral edema, a normal heart, three liters of ascites, no jaundice, no esophageal varices and a small liver (700 grams), which was cirrhotic. The biliary tract was normal.
